All incoming 7th graders for the new school year are required to have their immunizations up to date before they begin school.

Minnesota School Immunization Law 121A.22 requires that students entering 7th grade show proof of the following immunizations:

1-MMR (second dose)
2-Hepatitis B (series of 2 or 3 shots)
3-Td Booster (given after age 11)
4-Varicella (or history of chicken pox disease signed by MD or parent)

If your student has not received the Hepatitis B series, we recommend beginning it now to allow time for completion before 7th grade begins.

Students who cannot be vaccinated for medical reasons or whose parents are conscientiuosly opposed to immunizations may receive legal exemption. Please contact the nurse for details at

For families who need medical or dental care assistance, please contact the school nurse for details.

Updating Contact Numbers

We are in need of obtaining new phone numbers that you or your emergency contact people may have acquired since your child started school. If your child is ill and we are unable to reach you, we are at a disadvantage for helping them. If it is an emergency,
we may have to call 911.

If your child needs to take medication during the school day, for either a long or short duration, we need both the doctor's order and your permission within 2 days of beginning the medication, or we cannot give it. You may have the doctor FAX the order to us at (763) 971-4531.

We are grateful for the follow-through you have provided your children who have needed the vision or hearing screening. If you have received a referral for vision or hearing and are in need of financial aid,
please contact the

Health Office for assistance at (763) 971-4509.

to residents of Hennepin County who do not
have insurance, have insurance but have an out-of pocket cost for shots as well as other needs that covers the cost of immunizations and basic medical care.
